Port of the old mssql extension to newer PHP

A PHP extension that ports the old mssql_* functions from PHP 5.6 to PHP 7 and 8, built on FreeTDS. The README says it is for moving legacy apps off old PHP, not for new code, and it has only been tested on 64-bit Ubuntu with PHP 8.

- The behaviour changes from the original are written down: persistent links are removed, mssql_close becomes a no-op, and connections are freed when the last reference goes away. Someone porting code knows what shifted.

- The TDSVER environment variable can be set per connection, so a legacy app can talk to an old SQL Server without editing freetds.conf. That fits the problem a porter actually has.

- It warns when VARCHAR values or column names get truncated under pre-7.0 TDS. The old extension lost that data silently.

- Only tested on Ubuntu 64-bit with PHP 8. The tree shows no test suite or CI, so nothing checks the ported functions against the old behaviour, and a regression on another PHP version or platform would go unnoticed.

- Persistent connections are removed outright. An app that depended on pconnect-style reuse will see a different connection profile under load, and the README doesn't say what that costs.

- The build depends on FreeTDS, not Microsoft's driver, so the TDS version ceiling and the newer SQL Server login and encryption options are FreeTDS's to support. No LICENSE file appears in the visible tree, so reuse terms are unclear beyond CREDITS.txt.
